1. I hereby apply to be admitted as an ordinary member of Mount Pritchard & District Community Club Ltd (Mounties Group) and a member of the Breakers Country Club, subject to and effective from completion of the amalgamation of Mounties Group with Breakers Country Club. I agree to be bound by the Constitution and By laws of Mounties. I authorise the transfer of my breakers Rewards points from Breakers Country Club to Mounties Group in the amalgamation process.
2. I consent to receiving marketing materials, advertising materials and other offer materials from Mounties Group and companies in which Mounties Group has an ownership interest, including but not limited to material relating to birthday rewards, prize draws, promotions, entertainment, food, beverage, gaming machines, gaming and the Mounties Group Club Rewards loyalty program.
3. I understand that membership is not transferable and the fee, or any part thereof is not refundable. Perpetual membership is prepaid Club membership which will continue, subject to the Club's constitution and the Registered Clubs Act 1976
4. All Club Members over 18 years are entitled to participate in Mounties Group Club Rewards loyalty program (except for employees and contractors, who are subject to restrictions). I agree that my participation in the Mounties Group Club Rewards loyalty program is subject to the Mounties Group Club Rewards Terms and Conditions (as amended from time to time).

I agree that my participation in the Mounties Group Club rewards loyalty program is subject to the Mounties Group Club Rewards Terms and Conditions (as amended from time to time). I consent to the automatic renewal of my membership when I have sufficient Club Rewards Points.
